Unlike some other places where you won't get any consultation, our Principal Consultant Mr. Ambrose CK Ting will go through with you how to fill up the LPA Form 1, and get a lawyer to sign off as the "Certificate Issuer".

He will also brief your Donee on what are his or her responsibilities and guide them to go online to accept their Donee appointment.

Filling up of the LPA Form 1 plus explanation via Office of Public Guardian Online (OPGO) system using Singpass
Witness signature for your Donee(s) - guidance will be provided to teach them how to accept online

*If you require us to meet up with your Donee(s) to sign as witness, there is an additional charge of SGD30/- per location.

For example, if you have 2 Donees and they cannot come to the lawyer's office during office hours to sign, then I can arrange to meet up with them either after work or during the weekend to get their signature. If you can arrange for the two of them to be in the same location, then it is additional SGD30/- net. However, if I need to travel to two different locations to get their signature, then it will be SGD60/- extra.

Note: Currently, registration fee of S$75 is waived for Singaporeans to make an LPA Form 1. Singapore Permanent Residents will have to pay S$100 and foreigners pay S$250.

Step By Step Guide to getting your LPA Form 1:

Call 3116-2218 during office hours & ask for Mr. CK Ting of The Legacy Protector Pte Ltd and tell us your requirements.
Our Principal Consultant Mr. CK Ting will advise you on how many Donees you need and who they should be. He will also explain to you how to fill up Part 3A & Part 3B (Page 6) of the LPA Form 1.
Fix an appointment with our panel of lawyers (during office hours only, we have 3 locations to choose from: Peninsula Plaza, Adelphi & People's Park Complex)
Go to the law firm together with your Donee(s) and bring along ALL original NRICs.
If your Donee(s) cannot go to the law firm, we can arrange to meet separately at SGD30/- per location (see above).
You will get your LPA Form 1 within 15 minutes after the lawyer signs off as "Certificate Issuer"
We will help you to mail out your LPA Form 1 to the Office of Public Guardian (OPG).
The OPG will send you an acknowledgement letter within 1 to 2 weeks, and will wait another 6 weeks before they mail you back the original LPA Form 1.

LPA Form 2:

Any deviation from Form 1 is considered Form 2.
Allows the donor to state all his/her instructions specifically and in detail. Form 2 MUST BE drafted by a lawyer.

Note: Registration Fee * for Form 2 is S$200 for Singapore Citizens, $250 for Singapore PRs, and $300 for Foreigners.

(1) What is an LPA?

An LPA is a legal document which allows a person who is at least 21 years of age ('donor'), to voluntarily appoint one or more persons ('donee(s)'), to make decisions and act on his behalf as his proxy decision maker if he should lose mental capacity one day.A donee(s) can be appointed to act in two broad areas:- Personal Welfare; and- Property & Financial affairs or matters.

(2) What are the Benefits of an LPA?

The LPA allows you to appoint someone you trust to make decisions and act on your behalf should you become vulnerable when you lose the mental capacity to makeyour own decisions one day.By having an LPA in place, it also alleviates the stress and difficulties faced by your loved ones if you should lose mental capacity.

(3) What is the difference between an LPA and a Will?

A Will is a legal document which allows you to state the distribution of your assets after your death to the people you choose as your beneficiary. The Will does not allow you to appoint someone to act for you during your lifetime if you lose your mental or physical abilities.The LPA takes effect when you are alive but have lost your mental capacity. The LPA ceases to have effect upon your death. The LPA does not allow you to state the distribution of your assets after your death.In other words, LPA is sometimes known as the "Living Will" because it can take effect while you are still alive but loses mental capacity.A Will can only take effect upon death.

(4) What are the criteria to qualify someone to make an LPA?

The requirements for making an LPA are:

You must be at least 21 years old
You must have the mental capacity to make the LPA
You must not be an undischarged bankrupt if you wish to make an LPA for Property & Financial matters.

For the LPA to be valid, it must be registered with the Office of the Public Guardian.

(5) Why must I pay for LPA Form 1? I thought it is free for Singapore Citizens?

You are paying for the "Certificate Issuing Fee" for LPA Form 1.
The "Application Fee" for LPA Form 1 is waived for Singapore Citizens.
All Lasting Power of Attorney applications have to be witnessed and certified by a practicing lawyer, a registered psychiatrist or medical practitioner accredited by the Office of Public Guardian.

The certificate issuing fee is a professional fee that certificate issuers such as accredited general practitioners, psychiatrists and lawyers charge.
The LPA is an important legal instrument to be used when an individual is most vulnerable and without mental capacity to manage his/her own affairs. Therefore, it is important to have appropriate safeguards to protect the interests of the individual. The certificate issuer plays this role by certifying that the applicant is not making an LPA under due pressure or duress, and that he/she understands the nature and effect of the legal instrument. For this reason, the certificate issuer charges a professional fee for the service.
Our Fee of SGD180/- net (before promotion) is already inclusive of our lawyer's certificate issuing fee plus form filling and signing off as witnesses for your Donee(s).
